If they use Adium, they have to add the Custom Emoticon MSN Pack themselves, otherwise they won't be able to see them. You can't "force" their Adium to show them without it.

Personally I usually open Vienna, check everything (Which means 50+ new posts), close Vienne and open it hours later. I've seen something similar, with theme variants not loading until after you select the theme a second time. It seems to coincide with wether or not a group chat is active, ie. in the chat window. Do you see the same?

It is out of the scope for Adium developers to implement the protocol, but if you can get the Pidgin guys to implement it in LibPurple, then it would be pretty easy to add to Adium.

No, because that is the whole point of the feature. But you can just set it to a combo you don't use in any other applications, for instance by using two modifier keys.
