It may be a problem with Perian which handles .flv files.

.flv audio gets out of sync with video
I have seen this many times with QuickTime 7.6 and Leopard 10.5.6. Audio starts out in sync with video but gets out of sync. The same .flv file played with RealPlayer works fine. Played with VLC it is jittery and jumpy and pretty bad but the audio remains in sync.
