Perian does not read the m4v files; Apple's component does. Therefor, Perian isn't even used for reading the file format and thus cannot read the subtitles.

Perian cannot cause this, PERIOD. If you can't boot of the install disk, it's a hardware issue, not software. Send it in for repair.

Does the system pref for perian report it as installed? Did you quit the QuickTime Player before attempting to open the flv file? Is it really an flv file and not really something else?

Repair permissions. Most likely it cannot move the existing ones to the trash. You can also try deleting the components yourself, and then let the pref pane install.

Warning: iMovie is a bit persnickety about the files formats it opens. While QuickTime Player may be happy to open the file, there is no guarantee that iMovie will, even though it is now fully capable.

Uh... I have news for you; it isn't doing that in 1.1.1. In 1.1.1, in order to fix the updater, we accidentally broke it from running at all. So, you still have a version of 1.1 running there somewhere.
