If you mean to create your own, I'd say your best bet is to take one that is already made, download it, and click on it while holding 'ctrl', to chose 'show package contents'.
Then you can edit the ones already there, or replace them with the ones you already have, and then when you have replaced them, simply double click on the original icon, it will install them for you.

I think he means the MSN custom emotes. Currently you can't save them or send them you can only receive them.

Yes currently u can just save but u can't use the emoticons. Other ppl that's not using Adium can't see our emoticons. Any solutions for this?
