If anyone is interested, I've hacked together a pair of shell scripts that lets NCID - a network-based callerid package - work with contacts - a utility to provide access to the Address Book from the shell - to provide callerid with address book lookup of name and picture. They are available (with a screenshot of it in action) here:

I did not have a Windows box available, so macyac wasn't an option. NCID will run on pretty much anything - I run it on a linux box, although it can run natively on OSX.
