OK - I'm almost apologetic about mentioning this: it's a tiny, tiny issue - but because of that it may only require a tiny tiny fix :)
The Adium icon in my menu bar is 'seperate' from the OS X icons - in a way that's slightly annoying to hover over them. For instance - if i click on the clock, for a menu, and drift my mouse left, over volume, language settings, and displays; their menus appear - I reach Adium, and it doesn't. Likewise, if I click on Adium for a menu and drift right, I can't access the OS X utilities without re-clicking.
I realise this may be a 'feature' of the OS itself for which there's no workaround - in which case feel free to slap me down like a fool - but I've no other apps to test it with.

Rather than a feature is a no-feature. Apple forbides any third-party app to become a true menu extra, so we have to use the public API for the thing which as you pointed out has several limitations.

It'd be a separate application.. but would then fall nicely into a miscellaneous category on the xtras site which should exist at some point anyways.
